Before, During & After Your Clinic Visit: How To Prepare & What to Expect

Reminder, When Planning Your Trip and the Timing for Your Appointment:

Please remember that parking can be challenging so please allow for extra time to park if you are not being dropped off or using Valet Services.

What to Bring:

  • Your picture ID & Insurance Card
  • Insurance Referrals/Authorizations
  • List of current medications
  • Parking payment
  • Any MRI, CT scan and/or lab results related to your diagnosis. Bring disc and printed reports.
  • List of questions and/or notes about important information you want to be sure to discuss with your provider. 
    • To make the most out of your visit with your doctor, we encourage you to think about your most important questions and write them down ahead of time.

Topics to Consider Discussing with Your Provider: A Guide:

When deciding what you want to review with your provider, if you have concerns, please share them. If you’re not sure whether a symptom is related to your MS or Neuroinflammation disease, talk about it with your provider so that they can help you sort it out the best course of action.

Please remember, if at any time you do not understand what your providers are saying, you can ask them to explain or clarify.

Topics:

  • New or Worsening Symptoms: Any new or worsening neurological symptoms including, but not limited to weakness, changes in sensation, visual impairment, imbalance and incoordination, difficulty walking, falling, double vision as well as loss of control of bladder (leakage, urgency, or accidents), new onset constipation, and/or impaired thinking or memory.
  • Changes in medications and/or supplements
  • Major changes in your life that may be impacting your health or ability to access care
  • Your experience medications prescribed by our clinic (Do you have side-effects? Do you understand how they work or why we have prescribed them? Do you feel like it is helping?)
  • How you are coping overall (For example: signs of depression or anxiety)
  • Your safety in your home (Are you falling more? Do you need equipment to help you be as safe as possible in the bathroom?)

When You Arrive, Before You See Your Provider:

  • You will be welcomed by the Front Desk Staff.
  • Although there is a kiosk that can be used to check-in, all patients will need to check-in with the Front Desk Staff.
  • Our Medical Assistant will take you to a private patient area to take your vitals.
  • Before you meet with your provider, you will be taken to a private patient room. This is where your provider will meet with you.
    • If you brought a caregiver or someone to accompany you, they can join you in the patient room if you want them there.

During Your Visit/Time with a Provider:

  • You will be given time to discuss your concerns and questions
  • Your recent labs and imaging will be reviewed
  • A physical and neurologic examination will be performed to better understand your concerns
  • Your clinician will review your treatment plan and next steps in your care
  • Please note that since we are a teaching hospital a resident or medical student may be assisting in some of your visits.  If you are not comfortable with this, please let your team know.

Follow-up appointment times can vary from 30 minutes to 1 hour depending on your clinician’s schedule. Please be mindful of these time limits, and if you run out of time you may be requested to schedule a close interval follow-up appointment to discuss further concerns

 After Your Clinic Visit:

  • After your appointment is over, you will check-out at the front desk.
  • You will be provided your After Visit Summary. This document contains a summary of your health, the treatment plan that you and your provider have agreed upon, and a guide future care, including self-management tasks.
  • You are also encouraged to schedule your next follow-up appointment at this time.
  • We have an iPad available so you can complete a brief survey about the visit you just had as well. We appreciate your feedback and take each patient comment and rating seriously.
